Meals on Wheels

 

Dedicated volunteers deliver hot lunches to individuals age 60 and over who are homebound due to illness or disability and are unable to prepare nutritious meals.
Funds from state, federal and local governments cover part of the cost.  The remainder is covered by contributions from recipients of the meals, churches, civic groups, and their families.
Providing well balanced nutritious meals is one of the most important ways that Bay Aging can promote better health and independence.

 

Medicaid Personal Care

 

Personal Care Aides are provided for dressing, bathing, grooming, light housekeeping, meal preparation, and bedfast care.

 

Care Coordination

 

Bay Aging Care Coordination Program provides professionally certified Care Managers to assess older persons' living situations and to determine cost-effective options to meet their needs.  Fees are based upon the client's ability to pay.
Elder Care Management can help you if you are sixty years of age or older, and you are:

A person requiring coordination of multiple services

A person whose family lives outside the local area

bullet

A person already receiving services through Medicare or Medicaid, and needs additional assistance

bullet

A person with problems in such areas as physical environment, support systems, financial support, emotional and physical health

A person whose regular support system is not working or will not be available in the near future

bullet

A person living alone and unable to access the services available to you in the community

Adult Day Break Centers

Adult Day Break/Gloucester, 7066A Hayes Shopping Center - Open Monday through Friday 8:30 am to  4:30 pm.  Provides day care for adults in Gloucester and the surrounding counties.

Adult Day Break/Reedville, 454 Main St.- Open Monday through Friday, 8:30 am - 4:30 pm.  Provides day care for adults in the Northern Neck area.

Adult Day Break/Tappahannock, 1415 C Tappahannock Blvd. - Open Monday through Friday, 8:30 am - 4:30 pm.  Provides day care for adults in Tappahannock, Essex and the surrounding area.

 

Fancare

 

This program provides for free fans to senior citizens age 60 or older who have health conditions that require cooling.  A portion of these funds may be used to purchase air conditioners for the frail elderly.

 

 

Virginia Insurance Counseling Assistance Program

(VICAP)

 

The mission of the Virginia Insurance Counseling and Assistance Program is to educate, objectively counsel and assist consumers on medical benefit programs and problems with Medicare, Medicaid, medical bill paying and long term care insurance.  The VICAP officer serves the Middle Peninsula, Northern Neck and the Peninsula.

 

Home Care

 

Community based, Long Term Care provided to at-risk, frail, home-bound persons to assist them in remaining at home.

 

Home Maker and Personal Care services available.

 

Senior Patrol

Senior Patrol is a program that educates beneficiaries to take and active role in protecting the Medicare and Medicaid programs.  Volunteer activities include giving presentations to senior groups.

 

Legal Aid

Bay Aging works with Legal Services of Eastern Virginia, Inc. and Rappahannock Legal Services to provide legal assistance to protect and secure the rights, benefits and entitlements of persons 60 years of age and older with the greatest economic and social need, with particular emphasis on low income minority individuals, in the Northern Neck and Middle Peninsula.
