  • To Qualify for Program you
    • be over the age of 65
    • have income at or below 300% SSI and are likely to be denied Medicaid due to excess resources
    • have a caregiver that is showing difficulty meeting your needs
    • are somewhat dependent in some areas of your daily care and activities
    • have a health condition that requires at least monthly monitoring such as diabetes or heart disease.
  • Professional Staff
    • A Professional Care Coordination staff will work closely with you to develop a plan of care and arrange for your needs.
    • After a plan of care is created, you decide which service providers you want to meet your care needs.
